New Hire Paperwork and Benefits

New employees must complete the following forms (link below) and submit them to Human Resources at 2115 Grand Ave (Open Monday - Friday from 7:30 to 4:00 ). If you have not received a D51 Employee ID number yet, please leave that field blank.

The IRS has created a Tax Withholding Estimator and FAQ page to help individuals complete the current W-4 form.

If you are currently employed as a substitute, please only complete the paperwork linked below when moving to a regularly scheduled assignment.

From Substitute to Regular Position Paperwork Packet

Many of these forms are designed to be filled out on your computer. By completing them ahead of time and printing them at home, you can significantly reduce the amount of time you spend at Human Resources. 

When you bring your paperwork to HR, you will also need to bring:

Social Security Card* (If you don't have your social security card, or if your card does not show your current legal name, you must get a new card.)

Driver's License or other picture ID*

Voided Check or other approved document from your bank showing your account and routing numbers for direct deposit.  Documentation cannot be handwritten and must reflect routing numbers.

 *Forms of identification such as a social security card or driver's license must be the physical originals for purposes of the federal I-9 form.  No photocopies of pictures on your phone are acceptable.

If your position is eligible for benefits, please visit our Employee Benefits site for information about our D51 benefits plans, including medical, dental, vision, life insurance, and other voluntary products.  An email will be sent to your D51 email address  explaining when your benefits will be effective and directions to complete your enrollment online.  Please watch for this email.

Be prepared to have your picture taken for your D51 ID badge at Human Resources when submitting your new hire paperwork.
